Details: Autumn Leaf isÂ a transitional housing program serving seven women who are chemically dependent and their children. These women reside at Autumn Leaf after exiting inpatient treatment and receive six to nine months of intensive case management, which focuses on life skills, parenting, and self-sufficiency.
